at inception then Legal Evidence Title (LET) documents are required to release
the payout. Stamp duty in Rajasthan (Rs 200/-)

Any other
document which SBI Life in its discretion may require.
Additional
documents are required for death due to accident in addition to the requirements
mentioned above.

of assigning case for investigation & thus avoiding the delay in processing)

Hospital Treatment Certificate

Medical attendant’s certificate

Cause
of death certificate
o
Discharge summary and all past treatment papers
o
If
death is due to unnatural causes (accident / suicide / murder etc.) attested
copies of various police reports such as First Information Report (FIR), Police
Panchanama, Inquest Report, Post Mortem report. (In some cases, Police Final
Report or Magistrate’s verdict is also be required)

SBI
Life may call for any other document required for the assessment of the claim.
